PACE Social Worker (Onsite, 5-Month Temp)

Neighborhood Healthcare PACE in Riverside, CA seeks a Social Worker to manage a caseload of participants, applying a psychosocial perspective within the interdisciplinary evaluation and plan of care.

The role collaborates with the IDT, coordinates family conferences, and supports end-of-life planning, caregiver education, and discharge planning to ensure safe, person-centered care. This is a full-time onsite position lasting about 5 months with Mon–Fri 8am–5pm.
