The mission of PACE is to deliver person-centered care in the setting that’s most appropriate for each participant—whether that’s in their own home, with family, in long‑term care, or assisted living.

Centra’s PACE program is part of a comprehensive network of nearly 600 employed providers within Centra Health. Centra Health serves a population of 600,000 community members with 70 different outpatient and specialty practices across Central and Southern Virginia—from Lynchburg to Danville, Farmville, Moneta, and Bedford. CMG includes family practices, specialists, and surgeons in fields such as cardiology, neurosurgery, psychiatry, urology, and more. CMG also hosts the Lynchburg Family Medicine Residency, a training program for future family physicians.

Why Farmville?

Responsibilities
- Monday – Friday, 8:00 am – 5:00 pm; no weekends or holidays.
- Work with a dedicated interdisciplinary team—including providers, support staff, physical therapists, occupational therapists, dietitians, and social workers.
- See 5 patients per day—small volume.
- Call is once every 7 weeks, 5 pm – 8 am; triaged before reaching you, no out-of‑home calls.
- Build meaningful relationships with our elderly population.
Required Qualifications
Experienced providers in palliative, hospice, primary care, or long‑term care are encouraged to apply.
Qualifications
- Doctoral degree from an accredited Physician Assistant School.
- Current licensure by the State of Virginia as a Physician Assistant with ACLS (required only for LFM).
