PA Licensed Behavioral Health Case Manager

To support members facing behavioral health challenges, the full-time PA Licensed Behavioral Health Case Manager will provide telephonic engagement, develop personalized care plans, and coordinate with healthcare teams in a fully remote capacity within the Tri-State Area of Delaware, New Jersey, or Pennsylvania. Key Responsibilities: Telephonically engage members to identify barriers to care related to Social Determinants of Health (SDOH) Facilitate the development of care plans by identifying members' strengths, needs, and goals, providing follow-up as necessary Serve as a liaison within the member's healthcare team, especially in complex behavioral health situations that require care plan adjustments Required Qualifications: Active PA Registered Nurse (BCBA preferred), LCSW, LSW, or LPC required Minimum of 3-5 years post-licensure experience in an integrated care setting or healthcare environment Strong understanding of Social Determinants of Health and Motivational Interviewing techniques Ability to work independently and resolve complex behavioral, medical, and psychosocial issues Exceptional assessment and engagement skills with proficiency in telephonic and virtual communication
