Oasis Center seeks an Outreach Case Manager to support youth and families experiencing crisis, homelessness, or instability across Nashville. In this mission-driven, youth-centered role, you’ll build trusting relationships, complete assessments, and create service plans that connect young people to counseling, housing, education, and leadership programs. Working on a collaborative, trauma-informed team, you’ll provide field-based outreach, advocacy, and resource navigation, while maintaining accurate documentation and championing equity, inclusion, and youth voice in all aspects of your work.
