Community Care Physicians seeks an Outpatient Nurse Practitioner or Physician Assistant to provide patient-centered primary and specialty care in a busy, team-based practice. In this role, you will evaluate and treat adult and/or pediatric patients, manage chronic and acute conditions, order and interpret diagnostics, and coordinate care across our multispecialty network. You’ll work closely with physicians and support staff in a collaborative, physician-owned group that values quality, work–life balance, and long-term relationships with patients.
