Home Health Registered Nurse
Day Shift: 5x8-Hour (08:00 - 17:00)
This is a 13-week contract position.
The Home Health Registered Nurse will provide skilled home care visits to patients with diverse medical needs, including post-surgical recovery and chronic conditions. The role involves direct patient care and coordination with interdisciplinary teams to ensure optimal health outcomes.
Responsibilities:
- Conduct comprehensive patient assessments and develop care plans.
- Provide nursing care for patients with conditions such as cardiac, orthopedic, respiratory, and neurological issues.
- Administer medications and treatments as prescribed.
- Perform OASIS assessments and document care using BayCare Forms and scheduling tools.
- Manage up to 8 patient visits per day, ensuring high standards of care.
- Educate patients and families on health management and discharge planning.
- Maintain compliance with health regulations and safety protocols.
Required Experience / Certifications / Licensure:
- Minimum of 1 year of home health experience.
- Valid RN license in Florida or Compact state.
- BLS certification required.
- OASIS training preferred.
- Must possess a valid driver's license and have access to a car for home visits.
- COVID-19 vaccination required or approved exemption on file.
- Flu shot required or declination with mandatory mask wearing while at work.
