Unit notes profiles are sent directly to the manager. Need two supervisor references (charge accepted) at submittal; one within 12 months and one within 3 years worked. Copy of the driver's license is needed at time of submittal to validate legal name - Pulse must reflect the name on a government issued ID - no middle initial or middle name. Two years experience in a healthcare setting required. Required certifications: BLS and NPI number. Also needs a CA OT license. HH requires a current driver's license and proof of automobile insurance. We would consider a COTA with home health experience if there are no OT applicants. Four visits per day on average; combination of evaluations and revisits. Coverage area includes north-eastern Sac into Folsom and Woodland - Yolo county service area qualify for reimbursement.
