Oregon RN Care Navigator

To support kidney health management, the 100% remote Oregon RN Care Navigator will manage a caseload of members, facilitating high-quality care coordination, conducting health screenings, and collaborating with healthcare professionals to optimize patient outcomes. Key responsibilities Handle in and outbound calls to deliver exceptional service and educate members on kidney health and treatment options Engage members in HealthMap's Kidney Health Program while following up based on complexity and policy cadence Act as a liaison among patients, their support networks, and healthcare providers to coordinate care and resolve barriers Required qualifications Active, unrestricted RN license required Bachelor's degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree CCM certification preferred Three (3) years of experience in case management preferred Experience in a dialysis center or transplant center preferred
