Oregon Licensed Telehealth Nurse Practitioner

To support a growing telehealth practice, the part-time Oregon Licensed Telehealth Nurse Practitioner will manage high-risk patients remotely, providing clinical assessments, annual wellness exams, and coordinating care to prevent unnecessary acute admissions, working 4-6 days per month. Key responsibilities Assess patients' healthcare needs and provide clinical care and consultation via telemedicine Perform annual wellness exams and coordinate continuity of care under the direction of primary healthcare providers Collaborate with an interdisciplinary team to ensure holistic patient care and complete documentation within 72 hours of visits Required qualifications Current Oregon Nurse Practitioner license 2+ years of clinical experience as a Nurse Practitioner in relevant fields such as Primary Care or Gerontology Nurse Practitioner certification from either the ANCC or AANP Current DEA licensure / Prescriptive Authority required Active CPR/BLS Certification
