To ensure timely and appropriate care for members, the full-time Oregon Licensed RN Case Manager will perform case management activities including assessment, planning, implementation, coordination, and monitoring of treatment plans while working from home. Key responsibilities Conduct comprehensive assessments to identify individual member needs and develop tailored treatment plans Coordinate and implement case management activities to achieve set objectives and goals Monitor and evaluate the effectiveness of treatment plans, ensuring compliance with medical policies and standards of care Required qualifications Graduate of an accredited nursing program with a current unrestricted Oregon Nursing license Certification in case management (CCMC) required or ability to obtain within 24 months of hire Minimum of 3 years of recent hospital, home health, or acute care experience Proficiency in Microsoft Office applications and typing at least 35 wpm Strong analytical skills and ability to manage challenging customer interactions effectively
