To ensure timely and appropriate care delivery, the full-time Oregon Licensed RN Case Manager will perform case management activities, including developing and monitoring treatment plans, while working remotely. Key responsibilities Conduct comprehensive biopsychosocial assessments to identify member needs and develop individualized care plans Coordinate services across behavioral health, medical, and community systems to promote integrated care Engage members and families using motivational interviewing and trauma-informed communication to support treatment participation Required qualifications Current unrestricted Oregon license as a Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) Graduate of an accredited school of Nursing or Social Work Certification in case management (CCMC) required or ability to obtain within 24 months of hire Recent healthcare experience in acute care and/or skilled nursing care, including medical, surgical, and behavioral health knowledge Proficient with Microsoft Office applications and ability to type a minimum of 35 wpm
