Oregon Licensed Primary Care NP/PA

To provide virtual primary care through an innovative telehealth platform, the full-time Oregon Licensed Primary Care NP/PA will manage a panel of patients, conducting comprehensive assessments, developing chronic condition management plans, and collaborating with a multidisciplinary team to ensure coordinated care. Key responsibilities Deliver longitudinal virtual primary care visits via telehealth to a panel of patients Diagnose and manage chronic conditions, developing individualized care plans to optimize patient health Provide health coaching and patient education to empower patients in managing their health Required qualifications Active Advanced Practice Provider (APRN or PA) license in good standing in Oregon Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine Minimum 4 years of primary care, urgent care, or internal medicine experience, preferably in telehealth settings Experience managing chronic conditions and preventive care via telehealth Active BLS certification at time of hire
