Oregon Licensed Physical Therapist

To support patients on their musculoskeletal care journey, the full-time Oregon Licensed Physical Therapist Care Navigator will triage patients, guide clinical decision-making, and ensure access to appropriate care through high-touch telephonic engagement in a remote setting. Key responsibilities Guide clinical decision-making by conducting assessments and determining appropriate care pathways for patients Build trusted relationships with patients through telephonic engagement and provide ongoing support to navigate their care journey Coordinate care across the healthcare ecosystem by collaborating with patients, providers, and caregivers to ensure continuity of care Required qualifications Doctor of Physical Therapy (DPT), Doctor of Physiotherapy, or Master of Physical Therapy (MPT) Active, unrestricted Physical Therapist license in Oregon, with the ability to obtain additional state licensure as needed 5+ years of orthopedic or musculoskeletal clinical experience Strong clinical judgment and triage skills for assessing complex patient situations Experience in motivational interviewing and patient coaching to support behavior change
