Oregon Licensed Nurse Practitioner

To support a growing Senior Community Care program, the full-time Oregon Licensed Nurse Practitioner will provide virtual care and manage high-risk patients in various settings, collaborating with an interdisciplinary team while working remotely from home. Key responsibilities Manage care coordination for members via telephonic and video platforms, ensuring effective communication among caregivers and healthcare providers Conduct routine and comprehensive assessments, managing acute medical situations and chronic condition exacerbations Document medical information accurately and engage in advanced care planning conversations with patients and caregivers Required qualifications Active and unrestricted Oregon Nurse Practitioner license and ability to obtain licenses in other required locations Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program Board certification through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center 3+ years of clinical experience in practice Current, active DEA licensure or prescriptive authority, or ability to obtain post-hire
