To support a clinical engagement team, the contract MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and provide patient education while working remotely in Oregon with flexible hours between 8:00am and 8:00pm CST. Key responsibilities: Engage members telephonically and schedule pharmacist consultation appointments Utilize prescription claims data to generate clinical recommendations and assess medication regimens Document patient care notes and participate in departmental meetings to enhance clinical programs Required qualifications: Current and unrestricted Pharmacist license in the state of residence Strong knowledge of disease state management guidelines and MTM services 1+ years of experience working remotely within a cross-functional team environment Intermediate proficiency with Microsoft Word, Outlook, and Excel Bachelor's Degree or Pharm D
