Oregon Licensed Care Navigator

To support a growing remote care program, the part-time Oregon Licensed Care Navigator will manage patient physiological markers, conduct wellness calls, and collaborate with care teams to promote health behavior changes while working within the PST time zone. Key responsibilities: Manage physiological markers such as blood pressure, weight, and blood glucose with clinical appropriateness Conduct monthly wellness calls and lead collaborative discussions to define health goals with patients Provide preventive health and disease management education and coaching to enhance patient engagement in RPM programs Required qualifications: Unrestricted RN/LVN/LPN license in Oregon and a compact state 1-3 years of clinical experience in patient management or disease management Experience with Medicare patients and performing virtual visits or telephonic care management Ability to work independently with minimal direction and a strong interest in professional growth Spanish speaking is a plus
