Ops Human Service Counselor III -

Minimum Qualifications:
- Experience working in a 120-bed skilled Long-Term Care facility
- Knowledge of Medicaid/Medicare eligibility or insurance processes
- Excellent communication and interpersonal skills
- Fluent in the English language: reading, writing, speaking
- Basic skills in the use of Microsoft Office Outlook and Word
- Willing to work evenings and weekends as needed
- Willing to provide management and coordinate case management, treatment, referred counseling and follow up services for residents
- Florida Licensed Clinical Social Worker
- Successful completion of the employment screening process to include, but not limited to, a national background investigation and drug test
Preferred Qualification:
- Bachelor's degree
- Knowledge of Health Insurance Authorizations & Eligibility & knowledge of Medicare A
Performance Standards and Expectations:
- Works under the supervision of the Veteran's Home Administrator
- Develop and implement policies and procedures for social services
- Perform duties as backup to the Admission Coordinator when needed
- Provides management, coordination and oversight for the social services department
- Monitors participation of the veteran resident in programs/services designed to meet psychosocial, mental and emotional needs of the veteran
- Participates in Interdisciplinary Treatment team meetings to develop a care plan
- Provide knowledge of Medicaid benefits to families and residents
- Perform other duties as assigned
