Holy Cross Medical Group (HCMG) and Holy Cross Hospitalare seeking a skilled and compassionate Advanced Practice Provider to join our Oncology team. The ideal candidate will have experience or a strong interest in Survivorship care and will work collaboratively under the supervision of multiple Oncologists and other healthcare professionals to deliver high-quality, patient-centered care.
Key Responsibilities
- Conduct comprehensivesurvivorshipassessments including history, physical examinations, andinterpretation of diagnostic studies.
- Develop,implementand regularly update individualized survivorship care plans following completion of cancer treatment.
- Summarize treatment history (including surgery, chemotherapy, radiation, andimmunotherapy) andoutline follow-up care
- Coordinate transition from active oncology care to long-term survivorship.
- Monitor for cancer recurrence and secondary malignancies
- Oder and interpretappropriate surveillancelabs, imaging, and diagnostic tests.
- Identifyand managelate and long-term effects ofcancer and its treatment, including referral to rehabilitation, specialty services, or supportive therapies as needed.
- Screen patients for psychosocial or emotional impacts and provide counseling and referrals as needed.
- Counsel survivorson health promotion, disease prevention, and age/riskappropriate screeningsand vaccinations
- Provide patient andcaregivereducation.
- Maintain compliance with organizational policies, regulatory guidelines, and best practice standards.
Qualifications
- Graduate of an accredited Physician Assistant or Nurse Practitioner Program
- National Advanced Practice Registered Nurse Certification (APRN) OR national board certification as a Physician Assistant.
- Current Physician Assistant or APRN license in the State of Florida.
- AOCNP Certification
- BLS/ACLS
