Dates Needed:
11/2-11/6
11/9-11/13
11/30-12/4
12/7-12/11
12/14-12/18
/23
2027 Dates are welcome if provider can cover several of these weeks listed
Details/Requirements:
Monday-Friday, 8am-4:30pm
10 patients per day, few new patients, more follow-ups
Support: Dosimetrist, Physicist, Radiation Therapists, RN, Referral Coordinator
Creds: 30-45 days or less for Temporary Privileges
Active Indiana license or active IMLC
Board Certified
