Bluestone Physician Services is seeking a Nurse Practitioner or Physician Assistant to deliver on-site, team-based primary care within Assisted Living and Memory Care communities in and around Blaine, MN. You will join a growing practice with a flexible schedule supporting patients Monday through Thursday and optional Friday clinical or administrative duties.
You will manage a panel of about 200 patients, design individualized care plans, and collaborate with LPNs, RNs, and social workers.
