On-Call Hospice Care Manager RN

Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will assess patients, schedule and perform after-hours visits, and coordinate care with the Interdisciplinary Group to maintain continuity of care and compliance.

You will lead nursing teams during nights and weekends, manage documentation, and support regulatory readiness while upholding compassionate patient- and family-centered care.
