On-Call Hospice Care Manager (RN)

Gentiva is seeking an Afterhours RN Care Manager to provide high-quality, compassionate hospice care during nights, weekends, and on-call hours. The role ensures clinical oversight and coordination for patients and families, with on-site visits, assessments, and urgent after-hours support.

You will collaborate with physicians and the IDG, update plans of care, educate families, and maintain documentation while navigating regulations and compliance.
