On-Call Hospice Care Manager RN

Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours, delivering direct patient care during nights and weekends and maintaining clinical oversight.

You will work with the Interdisciplinary Group to coordinate care, update plans of care with physicians, educate families, and ensure accurate documentation while upholding infection control and compliance.
