Joining a dedicated team, the remote full-time Oklahoma RN Case Manager will manage utilization review processes, coordinate patient care transitions, and develop discharge plans while working days and every other weekend. Key responsibilities Completes comprehensive assessments of patients' clinical, psychological, and financial needs Recommends and coordinates timely transfers to appropriate levels of care based on clinical needs Communicates discharge care plans and collaborates with healthcare professionals to facilitate timely patient discharge Required qualifications 2 years of experience in clinical settings such as home health, inpatient, or clinics Current RN licensure in the State of Oklahoma or a multistate license from an eNLC member state BLS certification from the American Red Cross or American Heart Association within 30 days of hire Ability to operate a vehicle and possess a valid Oklahoma State Driver's License Experience with managed care and payer/provider requirements is preferred
