Oklahoma Licensed Post-Acute Liaison

To support post-acute communities and healthcare providers across Oklahoma, the part-time Weekend Oklahoma Licensed Post-Acute Liaison will conduct remote assessments and pre-admission screenings while collaborating with facility Admissions Coordinators to ensure a smooth admission experience. Key responsibilities Conduct pre-admission screenings and assessments to determine clinical, equipment, and transportation needs Develop and maintain strong referral development relationships with referral sources Maintain accurate records of daily activity and referral source interactions within the referral tracking system Required qualifications Current and unencumbered Oklahoma LPN or RN license Experience as a hospital liaison Experience in a skilled nursing or skilled rehabilitation setting
