Ohio Pharmacy Student Support Specialist

To support a growing interdisciplinary care team, the remote Ohio Pharmacy Student Support Specialist will conduct medication record reviews, map care teams, and summarize patient information while committing to at least 12 hours per week during the academic year. Key Responsibilities Review patient medical records to summarize prescribed medications and fill histories Identify and document active ambulatory care providers and specialists for seamless communication Summarize recent Admission, Discharge, and Transfer (ADT) events to identify transitions of care needs Required Qualifications Completion of at least 2 years (P2) of an ACPE-accredited Doctor of Pharmacy (PharmD) program or equivalent Familiarity with commonly prescribed medications (top 200 drugs) Understanding of traditional management and pharmacological treatment of common chronic conditions Proficiency in navigating complex database structures while maintaining HIPAA compliance Strong clinical writing skills for summarizing complex data for a multi-professional audience
