Providing comprehensive, family-centered care management services, the full-time Ohio Licensed RN Care Manager will conduct assessments, develop individualized care plans, and collaborate with interdisciplinary teams remotely to support pediatric and young adult patients with complex medical, behavioral, and social needs. Key responsibilities Conduct comprehensive, trauma-informed assessments of pediatric and young adult patients, focusing on medical, behavioral, and social drivers of health Develop individualized, evidence-based care plans that address physical, behavioral, and social health needs while incorporating family preferences Collaborate with interdisciplinary teams and community partners to coordinate preventive and post-acute care services Required qualifications Bachelor of Science in Nursing from an accredited program is required Current, unrestricted RN license in Ohio is required; may be obtained within 90 days of hire Current Basic Life Support (BLS) certification from the American Heart Association is required Minimum of 3 years of clinical nursing experience is required, with a preference for experience in pediatric care management Proficiency in MS Office and experience with EMR software, such as Epic, is required
