Providing nursing support to members with low-acuity, short-term clinical needs, the full-time Ohio Licensed Resource Nurse will conduct focused transitions of care assessments, deliver clinical education, and perform in-person clinical exams, all in a 100% remote setting. Key Responsibilities Conduct outreach to members during inpatient admissions and post-discharge for transitions of care assessments Perform comprehensive medication reconciliation and address quality gaps through proper documentation and coding Review charts and clinical data to identify members needing higher levels of care management or short-term interventions Required Qualifications Graduate of an accredited school of nursing (R.N.) 3+ years of nursing experience Strong critical thinking skills with sound clinical judgment Proficiency in using technology for remote member engagement Ability to apply Motivational Interviewing and Trauma-Informed Care principles
