To support a centralized Medication Refill Call Center, the full-time Ohio Licensed Practical Nurse will provide nursing care, perform health assessments, and collaborate with the healthcare team while working Monday through Friday from 9:00 a.m. to 5:30 p.m. Key Responsibilities Conducts initial and ongoing health status assessments and documents findings to inform care plans Documents and coordinates nursing interventions based on patient needs and collaborates with the healthcare team Participates in process improvement activities and acts as a patient advocate to enhance clinical quality and customer satisfaction Required Qualifications High School diploma or GED BLS - Basic Life Support certification from the American Heart Association LPN - Licensed Practical Nurse certification from the National Council of State Boards of Nursing Preferred 3-5 years of LPN experience Preferred minimum of 1 year of Primary Care experience
