To support high-quality palliative care delivery, the remote Ohio Licensed Palliative Care Provider will lead clinical care teams, coordinate with network providers, and analyze population-level data to enhance patient outcomes. Key responsibilities Lead internal and joint case rounds to ensure effective care plan development and execution Coordinate services with Network and Longitudinal Care Providers for optimal patient support Monitor and analyze network data to identify trends and implement necessary interventions Required qualifications Master of Science in Nursing (MSN) or Master of Science in Physician Assistant studies Five years of clinical experience in palliative care or hospice care Active Registered Nurse or Physician Assistant license in Ohio without board actions Board certification as an advanced practice nurse by a nationally recognized certifying agency Active Unrestricted DEA license
