• Barnet Dulaney Perkins Eye Center – Flagstaff (clinic with attached ASC)
• Southwestern Eye Center – Sedona & Cottonwood (clinics & ASCs)
• M&M Eye Institute – Prescott (clinic & ASC)

Our Northern Arizona locations include a clinic with attached ASC in Flagstaff, along with clinics & ASCs in Sedona & Cottonwood and Prescott. The region offers scenic landscapes with pine forests, historic sites and access to natural wonders like the Grand Canyon.
