About the Role
We are seeking a compassionate Certified Occupational Therapy Assistant (COTA) to join a collaborative rehabilitation team in a Skilled Nursing Facility. Working under the supervision of a licensed Occupational Therapist, you will help patients regain independence, improve functional mobility, and maximize their quality of life through individualized occupational therapy treatments.

We are seeking a compassionate Certified Occupational Therapy Assistant (COTA) to join a collaborative rehabilitation team in a Skilled Nursing Facility. Working under the supervision of a licensed Occupational Therapist, you will help patients regain independence, improve functional mobility, and maximize their quality of life through individualized occupational therapy treatments.
Responsibilities
- Provide skilled occupational therapy treatments under the supervision of a licensed Occupational Therapist.
- Implement individualized treatment plans to improve ADL performance, functional mobility, upper extremity function, cognition, safety awareness, and overall independence.
- Accurately document treatments using electronic medical records.
- Collaborate with the interdisciplinary team regarding patient progress and discharge planning.
- Educate patients, caregivers, and facility staff on adaptive equipment, home safety, and functional strategies.
- Maintain compliance with Medicare, state, and facility documentation standards.
- Participate in care conferences and team meetings as appropriate.
- Graduate of an accredited Occupational Therapy Assistant program.
- Current California COTA license (or eligibility).
- Current CPR/BLS certification.
- Skilled Nursing Facility experience preferred, but motivated new graduates are encouraged to apply.
- Excellent communication and teamwork skills.
- Positive attitude with a passion for patient-centered care.
- Complete documentation in Net Health per facility standards.
