Oasis Therapy Services is seeking an Occupational Therapist to provide rehabilitation services in a skilled nursing facility in Burr Ridge, Illinois.

Responsibilities
- Evaluate residents and develop individualized occupational therapy treatment plans.
- Provide treatment focused on activities of daily living, functional mobility, safety, and independence.
- Monitor residents' progress and modify treatment plans as needed.
- Complete discharge planning and provide recommendations for continued care.
- Educate residents, families, and caregivers on exercises, adaptive techniques, and equipment.
- Supervise Occupational Therapy Assistants when assigned.
- Complete accurate Net Health documentation and collaborate with the interdisciplinary care team.
Qualifications
- Active and current Illinois Occupational Therapist license.
- Graduate of an accredited Occupational Therapy program.
- Current CPR certification.
- Strong communication, organization, and documentation skills.
- Geriatric, skilled nursing, or rehabilitation experience is preferred.
