We are seeking a compassionate, skilled, and motivated Occupational Therapist (OT) to join our rehabilitation team on a PRN basis . In this role, you will provide high-quality, individualized occupational therapy services designed to help residents regain function, increase independence, improve safety, and achieve meaningful quality-of-life goals.

What You'll DoConduct comprehensive occupational therapy evaluations and identify residents' functional needs.
Develop individualized treatment plans based on clinical findings, physician orders, resident goals, and care plans.
Provide skilled therapy interventions to improve strength, mobility, coordination, balance, cognition, communication, activities of daily living, and overall functional independence.
Monitor resident progress and adjust treatment approaches based on clinical response and changing needs.
Complete accurate, timely, and compliant documentation, including evaluations, daily treatment notes, progress reports, and discharge summaries.
Collaborate with nursing, physicians, physical and speech therapists, social services, and other members of the interdisciplinary team.
Participate in care planning, utilization review, and interdisciplinary meetings as appropriate.
Educate residents, families, caregivers, and facility staff regarding therapy goals, interventions, safety strategies, and recommendations.
Promote resident dignity, safety, independence, and quality of life throughout the rehabilitation process.
Maintain productivity, documentation, and quality standards.
Support facility compliance with applicable clinical, regulatory, and documentation requirements.
Graduate of an accredited Occupational Therapy program.
Current and active Occupational Therapist license in the applicable state.
Strong clinical assessment, treatment-planning, and documentation skills.
Excellent communication and interpersonal skills.
Ability to work effectively with residents, families, caregivers, and interdisciplinary team members.
Long-term care, skilled nursing, or geriatric rehabilitation experience preferred.
Dependable, compassionate, and committed to providing high-quality resident care.
Ability to work independently while contributing positively to a collaborative rehabilitation team.
Ability to stand, walk, bend, reach, and perform other activities required during therapy sessions.
Ability to safely assist, transfer, position, and support residents as clinically necessary.
Ability to work effectively in a clinical and long-term care environment.
