If you're ready for a rewarding career experience, your journey begins here!Benefits:Competitive salaryDaily pay option availableFlexible scheduleFriendly work environmentGenerous paid time off (PTO) packageMedical, dental and vision insuranceSupplemental disability and life insurance options availableFlexible spending account401k availableCareer advancement opportunities / tuition reimbursementReferral bonus/incentivesResponsabilities:· Assessment and Evaluation: Perform comprehensive assessments of residents' functional abilities, with a focus on activities of daily living (ADLs) and instrumental activities of daily living (IADLs).· Individualized Care Plans: Develop tailored intervention plans to improve residents' independence in daily activities.· Section GG Data Collection: Ensure accurate and consistent collection of Section GG data for annual, quarterly, falls, significant change assessments, BIMS, PHQ 2-9, and other clinical data as needed.· Treatment: Administer OT interventions, including ADL training, adaptive equipment provision, and environmental modifications.· Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.· Documentation: Comfortable and knowledgeable with point of service (PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/manage care progress updates in EMRs (Net Health/PCC).· Interdisciplinary Collaboration: Collaborate with other rehab staff, including physical therapists and speech-language pathologists, for holistic resident care.Join us at Morristown Post Acute, and start a career that's not just about work, but about making a meaningful difference in the lives of others.
