Position Summary :

Responsible for providing services to both inpatients and outpatients, including assessment, treatment, planning, implementation and related documentation.

Position Requirements :

  Graduate of approved School of Occupational Therapy.

• n

  Licensed as an Occupational Therapist in the State of Illinois.

• n

  Holds and maintains requirements for credential of "Registered" as issued by NBCOT

  n

• n

  Certified in CPR through the American Heart Association.
