Responsibilities
- Assess resident needs and develop individualized treatment plans to achieve goals
- Implement and modify treatment plans to address physical, cognitive, and emotional needs
- Collaborate with the interdisciplinary team to ensure coordinated, relationship-based care
- Educate residents and families on therapeutic exercises and techniques
- Monitor and report on resident progress and outcomes
- Maintain accurate, up-to-date patient care records
