Title: Occupational Therapist - Occupational Therapy
Requirements:
- Current Occupational Therapist license in the state of practice
- Graduate of an accredited Occupational Therapy program
- Prior skilled nursing facility or geriatric rehabilitation experience preferred
- Strong clinical reasoning, communication, and organizational skills
Roles & Responsibilities:
- Evaluate residents' functional abilities, including activities of daily living, mobility, cognition, and psychosocial factors.
- Develop and implement individualized treatment plans based on evaluation findings and physician orders.
- Provide direct occupational therapy services in accordance with state and federal regulations and facility policies.
- Monitor and document resident progress, adjusting treatment plans as necessary to meet goals.
- Educate residents, families, and caregivers on strategies to promote safety and independence.
- Recommend adaptive equipment and assistive devices as needed.
