Occupational Therapist/Occupational Therapy Assistant
Blue Stone Therapy has partnered with skilled nursing and senior living communities across the Midwest to deliver individualized, high-quality physical, occupational, and speech therapy services.
Location: Cherokee, IA. We are seeking an Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA) to provide rehabilitation services to adult and geriatric patients at Accura HealthCare of Cherokee.
Responsibilities
- Provide skilled occupational therapy services to adult and geriatric patients in a skilled nursing and post‑acute rehabilitation setting
- Perform patient evaluations and develop plans of care (OT) or implement treatment programs under the direction of a supervising Occupational Therapist (COTA)
- Deliver therapy focused on activities of daily living (ADLs), upper extremity function, coordination, cognitive skills, and safety awareness
- Monitor patient response to treatment and adjust interventions in collaboration with the therapy team
- Maintain appropriate patient schedules per established guidelines
- Complete timely and accurate documentation to support quality care and reimbursement requirements
- Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
If multi‑site, this position will serve multiple partner communities including Cherokee Specialty Care and Accura HealthCare of Aurelia, with travel between locations as needed.
Detailed Duties
- Perform occupational therapy evaluations and develop individualized plans of care
- Provide skilled occupational therapy treatment focused on functional and measurable outcomes
- Address ADLs, functional mobility, cognition, strength, positioning, and discharge planning
- Supervise Occupational Therapy Assistants in accordance with regulatory and company standards
- Maintain appropriate patient schedules per established guidelines
- Complete timely, accurate documentation to support quality care and reimbursement requirements
- Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
Qualifications
- Graduate of an accredited Occupational Therapy or Occupational Therapy Assistant program
- Current OT or COTA license (or eligibility for licensure) in the applicable state
- Experience in skilled nursing, long‑term care, or post‑acute rehabilitation preferred
- Strong clinical reasoning, communication, and organizational skills
- Commitment to patient‑centered care and professional standards
