In this role, you will evaluate patients, develop and implement individualized treatment plans, and provide ADL, functional mobility, and cognitive-perceptual training across acute, outpatient, and rehab settings. You’ll collaborate closely with an interdisciplinary team, educate patients and families, and document care in the EMR.
