- Team of 4 Ob/Gyns and 3 APRNs
- Two practice locations in Lakeland, FL
- Admitting hospital: Lakeland Regional Health Medical Center
- Lakeland Regional Health Medical Center has an 8 Room OBED, 33 Post-Partum Rooms, 7 Antepartum Rooms, and 17 Labor and Delivery Rooms
- Lakeland Regional Health Medical Center has a Level III NICU with 33 rooms and Neonatologist in house 24/7
- MFM is onsite at Lakeland Regional Health Medical Center

Requirements
- MD/DO
- Unrestricted State Medical License
- Unrestricted DEA License or ability to obtain
- OB/GYN BC/BE (ABOG, AOBOG)
- Board certification required within 5 years of eligibility
- BLS
