- Candidate must do both Obstetrics and Gynecology
- MD/DO from an accredited medical program
- Physician licensed or eligible for license to practice medicine in Illinois
- Board certified or board eligible in OBGYN
- Licensed by the Federal Drug Enforcement Administration
