To support a growing digital health practice, the remote NY/CA Licensed Nurse Practitioner will provide personalized patient care, manage virtual consultations, and collaborate with a care team while working four days a week with one day dedicated to additional patient communication and projects. Key responsibilities See patients virtually using the technology platform provided by the practice Work closely with a care team, including a Health Coach, to optimize patient outcomes Participate in patient care program development and assist in resolving patient inquiries Required qualifications Active NY/CA Nurse Practitioner license or active NY/CA Physician Assistant license Active DEA license 3+ years of experience providing primary care via telemedicine or in-person 2+ years of clinical practice post-residency 2+ years of experience practicing functional medicine
