The Intake Coordinator is a Registered Professional Nurse who takes referrals and processes appropriate home care services. To maintain open lines of communication with referral sources and within departments.
Requirements:
- Current NYS RN License
- Allscripts a MUST
- CHHA experience a MUST
- Computer literacy; experience with Home Care Home base a MUST
- Excellent communication skills
- Highly organized, possess a strong attention to detail
