Are you an experienced Registered Nurse with a background in Home Health, Medicare compliance, and OASIS assessments ? We're looking for a dedicated RN Clinical Manager to lead clinical operations, support care teams, and ensure the delivery of high-quality patient care.
Key Responsibilities:
- Oversee and coordinate direct patient care services.
- Review patient assessments, reassessments, and Plans of Care (POCs).
- Ensure compliance with Medicare Home Health regulations and OASIS documentation requirements.
- Coordinate care with clinicians, physicians, patients, and caregivers.
- Supervise, mentor, and evaluate clinical staff.
- Participate in recruitment, onboarding, training, and staff development.
- Review Start of Care (SOC), Resumption of Care (ROC), and recertification documentation.
- Promote clinical excellence, regulatory compliance, and positive patient outcomes.
Required Qualifications: Graduate of an accredited School of Nursing
Active Registered Nurse (RN) license
Current CPR certification
Minimum of 2 years of RN experience
At least 1 year of management experience in home care, hospice, or a similar healthcare setting
Required: Medicare Home Health experience, including hands-on OASIS assessments, documentation, and compliance
Valid driver's license, auto insurance, and reliable transportation
