About the Role
As a Nurse Practitioner or Physician Assistant, you'll conduct annual in-home assessments, identify care gaps, educate patients, and collaborate with primary care providers. This is a non-prescribing, field-based position focused on preventive care.
Qualifications
- NP: ANCC or AANP certification
- PA: NCCPA certification and state licensure
- Active, unrestricted license
- Valid driver's license and reliable transportation
Preferred
- 1+ years of clinical experience
- Home health, primary care, or geriatric experience
- EMR proficiency
Role Details
- Location: Yakima, WA
- Schedule: Flexible 24-40 hours/week
- Work-Life Balance: No on-call, weekends, or holidays
