ChenMed is seeking a Nurse Practitioner, Bridging Care Provider to join our clinical operations team. This role supports patients in ChenMed medical centers virtually and in home settings, under the Regional Chief Clinical Officer, training new PCPs and providing bridging care.
The NP will perform geriatric assessments, exams, diagnoses, treatments, education, and referrals, with strict documentation to ChenMed standards.
