Alignment Health is seeking a Care Anywhere (CAW) NP/PA to perform in-home assessments and deliver comprehensive medical and social support to our patients in the Sacramento area.
As part of a multidisciplinary team, you will conduct health screenings, review medications, identify needs, and coordinate with PCPs and specialists to optimize care while reducing hospitalizations. A CA NP/PA license and experience with geriatric populations are preferred.
