To support an innovative telehealth program, the full-time DC Licensed Diabetes Nurse Practitioner will manage patient care through a structured 14-week program, conducting virtual visits, adjusting medications, and developing personalized care plans for individuals with diabetes. Key responsibilities Conduct weekly follow-up visits and provide education to patients living with diabetes Collaborate with a multidisciplinary team to deliver coordinated, patient-centered care Assess, diagnose, and treat patients, adjusting medications and interpreting glucose data as needed Required qualifications Minimum of 1-2 years of diabetes management experience as an advanced practice provider Experience with continuous glucose monitoring (CGM), insulin pumps, and automated insulin delivery (AID) systems is highly desirable CDCES and/or BC-ADM certification preferred but not required Strong clinical judgment and critical thinking skills Exceptional interpersonal and telehealth communication skills
