NURSE PRACTITIONER – PRIMARY CARE

POSITION SUMMARY
Woodlands Medical Specialists is seeking an experienced and patient-focused Nurse Practitioner (APRN/NP) to join our growing primary care practice in Pensacola, Florida. The Nurse Practitioner will provide comprehensive outpatient primary care services to adult patients, including preventive care, evaluation and treatment of acute conditions, management of chronic diseases, medication management, patient education, and coordination of care. The ideal candidate will work collaboratively with physicians, clinical staff, and the care-management team while supporting the practice’s commitment to high-quality, patient-centered, and value-based care.
ESSENTIAL RESPONSIBILITIES
- Conduct comprehensive patient histories and physical examinations.
- Diagnose and treat acute and chronic medical conditions within the practitioner’s scope of practice.
- Develop and implement individualized treatment plans.
- Order, interpret, and follow up on appropriate laboratory and diagnostic studies.
- Prescribe medications and treatments in accordance with Florida law and practice protocols.
- Provide preventive healthcare services, including health screenings and immunizations.
- Manage common chronic conditions, including diabetes, hypertension, hyperlipidemia, COPD, asthma, heart disease, and other primary-care conditions.
- Perform appropriate office-based procedures within the practitioner’s training, competency, and scope of practice.
- Provide patient counseling regarding medications, nutrition, lifestyle modification, disease management, and preventive health.
- Review and appropriately address laboratory, imaging, and diagnostic results.
- Maintain accurate and timely documentation in the electronic medical record.
- Communicate effectively with physicians, medical assistants, nurses, specialists, and other members of the healthcare team.
- Coordinate referrals and follow-up care with specialists and other healthcare providers.
- Support appropriate closure of preventive and chronic-care quality gaps.
- Participate in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs consistent with Woodlands Medical Specialists’ policies and procedures.
- Support the practice’s participation in ACO and other value-based care initiatives.
- Work collaboratively with clinical and revenue-cycle teams to support accurate documentation and coding.
- Participate in quality-improvement initiatives and clinical audits as requested.
- Maintain compliance with applicable federal and Florida healthcare laws, regulations, and professional standards.
- Participate in provider meetings, continuing education, and other practice activities as required.
QUALIFICATIONS
- Current and unrestricted Florida Advanced Practice Registered Nurse (APRN) license.
- National certification as a Nurse Practitioner through an appropriate recognized certifying organization.
- Appropriate prescriptive authority and credentials required to practice in Florida.
- Current DEA registration, or ability to obtain one.
- Current BLS certification.
- Graduate of an accredited Nurse Practitioner program.
- Primary care experience preferred.
- Experience with electronic medical records preferred.
- Experience with Medicare, Medicare Advantage, Medicaid, and commercial insurance populations preferred.
- Experience with chronic disease management and preventive care preferred.
